II. Provisions And Agreement

I intend to sponsor/supervise student projects to be entered in science-o-rama!

I am confirming my agreement with each of the following provisions of the Science Project Grant by checking each corresponding box:
I understand that the Synopsys Outreach Foundation expects and anticipates that acceptance of the Grant, and my corresponding agreement to participate in this program, will result in my providing direction, guidance and assistance, on an ongoing basis, to those students who intend to participate in science fairs;
I understand that payment of the Materials Grant portion of the Science Project Grant will be made payable to my school;
I will work with other participating teachers at my school to identify a SCHOOL CAPTAIN to serve as a point of contact.
My school is a public school, or a private school with 501(c)(3) non-profit tax status (check with school administration if unclear);
My school is a partner with a Synopsys Field Office Corporate Giving Committee.

III. Grant Requests

I am a teacher and I am requesting a Science Project Grant to assist me in working with students developing science projects.
